Itachi was actually suffering from a respiratory disease which he was diagnosed with sometime after he joined the Anbu. Even though he continued to train and master his sharingan and genjutsu, this sickness limited his physical prowess and stamina. This is why every time he used his mangekyou sharingan he would be gassed immediately after. He never really showed it though, since he never lost a fight and always advanced until the end. The fight with sasuke doesn’t count as a loss for the record, he could’ve ended sasuke easily if he really wanted to. However, itachi wanted to give sasuke the satisfaction of killing him, since itachi massacred the uchiha clan (with good reasoning). Itachi knew he was dying and decided to join the akatsuki so it would be easier for sasuke to find him and hate him even more. Itachi intentionally wanted to die by sasukes hand to atone for his sins, which is why he left sasuke on his own to grow stronger and pursue him. He also mastered genjutsu like tsukoyomi so that he could conserve his stamina by avoiding direct confrontations. If Itachi was not handicapped by this disease it’s debatable that he could have been the strongest uchiha. He was already insanely skilled and the youngest uchiha to graduate from the academy. Imagine if he got to grow up completely healthy and continue to get stronger (maybe get EMS).. he would be unstoppable.

He overused the Mangekyo and it had a negative effect on him. We saw that when Madara took Izunas eyes ; or rather Izuna gave his eyes to Madara, Madara seemed to be in bed as if he was in great pain and going blind. So when you overuse the eye chances are its gonna have a really negative effect on your body. Obito never had to worry about this because half his body was Hashirama cells.
Itachi being in the Akatsuki would have always come across opponents that he had to use the MS on. Tracking down and capturing a tailed beast aint something you can do with a 3 tomoe sharingan. In the case of opponents as skilled as kakashi, itachi knew having a drawn out fight with him using his 3 tomoe sharingan would probably end in a stalemate and reinforcements would also arrive so best bet was to use Amaterasu, Susanoo or MS genjutsu. Any of which would make Itachi win the fight in like seconds. So during his time in the Akatsuki he would have come across opponents strong enough to make him want to end the fight before it begins i.e using an ability associated with his MS.
If Sasuke didn’t get Itachi’s eyes im betting he would have begun to suffer the same way itachi did. Remember obito gave Sasuke Itachi’s eye drops. Likewise had itachi taken Sasuke’s eyes he would no longer be burdened by the effects of the Mangekyo just as Madara did. This could be false but im pretty sure the Illness is 100% tied with the eye so upgrading to the EMS should solve this problem. Makes me wonder why itachi never used Fugaku’s eyes.

Hello, as someone who’s a pre-med student in hopes to be a doctor someday and judging by Mahesh and Faiq’s response, Itachi’s illness most likely would’ve been a form COPD (chronic obstructive pulmonary disease). From what I can gather he most likely had chronic bronchitis(extreme inflammation of the bronchial tubes in the lungs which is responsible for delivering air to and from the lungs) turned into emphysema(over inflation of the alveoli/air sacs, meaning the tissue in his lungs have been destroyed). Emphysema is progressive meaning the disease gets worse over time til the person is dead. Our boi had 0 chance of survival, what made him the great ninja he was ultimately killed him. Which is a shame bc all he wanted was peace but like they say “kindness kills.” 🙁
Edit: So naturally being the curious person and detective I am I continue to look further, I also speculate that he could’ve ultimately died of heart failure. After all he was coughing up blood as he was dying. Which most likely was due for dramatic effect but could be explained by a heart disease, I’m thinking coronary artery disease or cardiomyopathy. But chronic bronchitis can also result in coughing up blood and is a common cause.
Seriously though, it was never mentioned what he was suffering from. But I suspect it is a respiratory disease, similar to kimimaro’s sickness. Anyways, kishimoto found it relevant to kill of pure prodigies (meaning prodigies of prodigies) like itachi, minato and kimimaro, because they were so well written they were virtually impossible to kill, even for kage, level ninjas. So the best way to dispose of them was through an unknown sickness or self sacrifice.
The guess to Itachi’s disease is most commonly MPA or Microscopic Polyangiitis. This seems to be a pretty common theory among people as most of his symptoms are textbook. MPA is a very rare auto immune disease that causes antibodies to attack blood cells causing them to bleed as they become damaged and inflamed. MPA also attacks the vital organs of the body and causes bleeding, coughing, and fatigue which Itachi shows throughout the series. Madara at one point also tells Sasuke that Itachi took various medications to stay alive which is consistent with how MPA is treated.

However, all we know is that due to the mysterious disease, he was coughing blood similarly to Kimimaro. Therefore, we can in the very least ascertain that it was a respiratory disease.
